Request for Proposal (RFP)

This RFP includes the forms and information required for a proposal to be considered for funding. The proposal deadline is January 22, 2008.

The following are required as part of your proposal from your organization:

• Cover page for Grant Proposal (form)
• Abstract Form
• Biosketch Form
• Budget Form
• Project Description as outlined in the RFP
• Financial Information as outlined in the RFP
• Proof of non-profit status 
• Seven copies of proposal 
• Electronic version of proposal
